We have more patients who need physical therapy than we have room in our daily schedule to provide. When a patient does not show up for their appointment or cancels too close to their scheduled time, we are unable to fill this appointment time with another patient who desperately needs physical therapy. This policy is our attempt to ensure that both you and our other patients receive the physical therapy that you need.
Broken Appointments: Patients are only allowed ONE broken appointment in a 30- day time period.
Broken appointments are any time you are scheduled for an appointment, and you do not show for that appointment.
Late cancellations are considered broken appointments. If you need to cancel your appointment, we ask that you please call us at least 24 hours before your appointment time.
Late arrivals are also considered broken appointments. If you do not arrive within the first 5 minutes, there will be a $10 late fee. Anything after 5 minutes of the start time of your appointment, you will need to reschedule.
Appointment Confirmation: You must confirm your appointment no later than the business day before. Our practice closes at 6:00pm. It is your responsibility to confirm. This will be considered a broken appointment.
If for any reason, a patient misses their appointment or cancels late for a second within a 1-month period, they will need to be re-evaluated with Dr. Montes.
We value you as our patient and need your cooperation with keeping appointments so that we can provide your care. Missing or late canceling an appointment means we are unable to fill this appointment time with another patient who desperately needs care. Our policy requires:

Timely Cancellations: If you need to cancel or reschedule your appointment, you must give us at least 24 hours’ notice. Cancellations made with less than 24hours’ notice will be considered a missed appointment, and a $20.00 fee will be applied to the next visit.
On Time Arrivals: If you do not arrive within the first 10 minutes, there will be a $20 late fee. Anything after 10 minutes of the start time of your appointment, you will need to call and therapy maybe decreased based on how late you are to arrive. This will be considered a missed appointment.
Compliance: Patients are only allowed ONE missed appointment in a 30-day period. After the second missed appointment, you will need to schedule a re-evaluation with Dr. Montes.

The staff at Baseline Animal Rehabilitation Center (BARC) spends a lot of time with your pet while they're here for their physical therapy session. We would love to share this time with you by posting pictures and videos on our website, facebook or other networking and communications venues. However, before we can post any pictures or videos of your pet, we'd like you permission.
I hear by permit Baseline Animal Rehabilitation Center to use pictures and videos taken at BARC, in which my pet/s may appear, for purposes of communications and marketing about Baseline Animal Rehabilitations Center. The results are considered the property of BARC and may not be sold or reused without express consent of Baseline Animal Rehabilitation Center.
